Risks of pregnancy in couples with a history of infertility

Couples who have infertility have greater risks with pregnancy even if they conceive on their own without any treatment. Every couple with infertility should review this consent even if they are not proceeding with treatment.

  • The following consents are for patients who intend to donate their extra embryos during their IVF cycle. These consents are for review only. You must sign consents in the office with Dr. Morris or the nursing staff.

  • Initial Embryo Donation Agreement Revised 5/25/2005 This consent is signed before the IVF cycle begins.

  • Final Embryo Donation Agreement Revised 5/25/2005 This consent is signed after the embryos have been created and frozen for donation.

    Egg Donation Consents (for the Egg Donor)

    These are the consents for the woman who is donating eggs in a fresh (non-frozen) cycle only. These consents are for review only. You must sign consents in the office with Dr. Morris or the nursing staff.

    Consents for an embryo thaw cycle

    These are the consents for attempting pregnancy from the thaw and transfer of previously frozen embryos. These consents are for review only. You must sign consents in the office with Dr. Morris or the nursing staff.
