Risks of pregnancy in couples with a history of infertility

Couples who have infertility have greater risks with pregnancy even if they conceive on their own without any treatment. Every couple with infertility should review this consent even if they are not proceeding with treatment.

In vitro fertilization (IVF) consents

These are the consents for performing a fresh in vitro fertilization (IVF) cycle with your own eggs. These consents are for review only. Actual consent forms will be sent from the office to sign online.

Embryo donation

The following consents are for patients who intend to donate their extra embryos during their IVF cycle. These consents are for review only.

Egg Donation Consents (for the Recipients)

These are the consents for performing a fresh in vitro fertilization (IVF) cycle with eggs from an egg donor. These consents are for review only.

Egg Donation Consents (for the Egg Donor)

These are the consents for the woman who is donating eggs in a fresh (non-frozen) cycle only. These consents are for review only.

 

Consents for an embryo thaw cycle

These are the consents for attempting pregnancy from the thaw and transfer of previously frozen embryos. These consents are for review only.
